Volunteer Services and Nursing Care.
Hospice of Gladwin Area, Inc., utilizes a network of trained volunteers to provide supportive services to the patient and family.  The services provided by our executive director and volunteers are provided at NO COST TO THE PATIENT AND CAREGIVER(S) BEING SERVED and may include one of more of the services listed under services on this page.
In addition to the services listed, Hospice of Gladwin Areas, Inc. has collaborations with several area health agencies in order to assist in providing professional nursing services.


Authorizations
The patient/caregiver(s) will be asked to sign two (2) authorizations upon admission into the care program of Hospice of Gladwin Area, Inc.  These are:

1.  INFORMED CONSENT - Giving permission for Hospice of Gladwin Area, Inc. to provide services.

2.  RELEASE OF MEDICAL INFORMATION AUTHORIZATION - Giving permission to the attending physician and/or nursing agency to release medical information to Hospice of Gladwin Area, Inc.

Services
Services of Hospice of Gladwin Area, Inc. are designed to support the terminally ill patient and caregiver(s) who have chosen to have the patient live out the final days in the comfort and familiarity of the home setting, surrounded by family and loved ones.  To assist in this care, personnel from Hospice of Gladwin Area, inc. are available on a 24-hour "on call" basis, seven days at week.

During office hours, our volunteer office staff can be reached to assist the patient/family.  After office hours and on weekend, our answering service will answer our calls and put persons in need in contact with the appropriate Hospice personnel.  The number to call during and after office hours is the SAME.
